Need a copy of your health records?

Federal law entitles you to be able to get a copy of your records from your health care provider. We make it as easy as possible—just fill out and submit the appropriate form to get started. For your protection, you may receive a phone call confirming your request.

Get your medical records

If you would like a copy of your Student Health Center medical records, or would like for it to be sent to another provider, complete our online Health Center Authorization for Release of Information form.

CAPS Release of Information form

If you would like your CAPS/SACS records to be sent to another provider or office, complete this Release of Information form.

Consent for Medical Treatment of a Minor

If you’re under 18 we need to have your parent or guardian’s permission to treat you at the Health Center. Make sure they fill out the online Consent for Medical Treatment of a Minor form.
